A US Air Force Air Education and Training Command Accident Investigation Board Report has found that pilot error was to blame for a fatal crash involving a T-38C Talon near Dannelly Field in Montgomery, Alabama, on February 19, 2021.
The report, released on October 10, identifies the aircraft involved as 68-8099 ‘CB’ from the 50th Flying Training Squadron (FTS) ‘Strikin Snakes’, 14th Flying Training Wing (FTW), at Columbus Air Force Base (AFB), Mississippi.
At 2240hrs Zulu (Z) (1640hrs local time) on February 19, 2021, a T-38C - tail number 68-8099, callsign ‘Slash 20’ - impacted the terrain short of Runway 28 while performing an instrument approach at Montgomery Regional Airport (Dannelly Field), Montgomery, Alabama. The Mishap Instructor Pilot (MIP) was a First Assignment Instructor Pilot (FAIP) and the Mishap Student Pilot (MSP) was a Japanese student pilot. Both pilots were assigned to the 14th FTW and flew with the 50th FTS at Columbus AFB, Mississippi.
